Provides advocacy, information and support to individuals that are at risk and/or institutionalized to move to the most integrated setting. Assists with coordinating discharges from institutions to community based living. Associates Degree in Human Services or related field and an in-depth knowledge of community based services and public benefits. A strong commitment to promote and advocate for the full integration of individuals with disabilities. Creativity and comfort in dealing with a wide range of diverse individuals and situations.
