Provides physician ordered physical therapy services, which include assessment, planning, developing, and implementing an individualized patient therapy plan. Coordinates the physical therapy plan with the interdisciplinary team care plan.
BS or MS degree from an accredited Physical Therapy school with current NYS licensure. Minimum of 2 years experience in an acute care facility and/or an acute intermediate rehabilitation facility. Possession of a valid NYS motor vehicle operator’s license. Pre-employment physical examination and an annual health assessment as require by New York State Health Department
