Our client, a leading home health care provider, is seeking qualified Physical Therapists in the Rochester Metro Area. The Physical Therapist provides physician ordered physical therapy services, which include assessment, planning, developing, and implementing an individualized patient therapy plan. Coordinates the physical therapy plan with the interdisciplinary team care plan. Essential Responsibilities/Accountabilities: 1. Provide physician ordered physical therapy services. 2. Assure orders are renewed, per schedule, or as patient’s response to current orders change. 3. Is responsible for completing, in a timely manner, all documentation including initial assessment, visit notes, renewal orders, verbal orders, and discharge summaries. 4. Identify patient short/long term goals in terms of functional outcomes. Reflect progress or lack of goal achievement in all charting. Confer with physician and/or interdisciplinary team regarding patient response to therapy. 5. Evaluate home environment for changes needed to increase patient’s independence. Recommend equipment and assistive devices (ramps, bathroom aides, adaptive wheelchair) as appropriate. 6. Instruct the patient and caregivers in therapeutic exercise, which may be used to improve the patient’s functional status. 7. Provide gait evaluation and ambulating training. Select and instruct in the use of various assistive devices (orthotic appliance, crutches, walker, cane, etc.). 8. Treat patient’s physical deficits using clinically accepted therapeutic exercises. Specify technique used in the plan of care. 9. Evaluate and instruct patient and caregivers in safe transfers (bed, chair, commode, etc.) using appropriate body mechanics and equipment (sliding board, hoyer lift, trapeze, bath bench, etc.). 10. Attend scheduled interdisciplinary staff meetings and actively participate in treatment planning. 11. Participate in committees and program development, as indicated. 12. Consistently demonstrates high standards of integrity by supporting the Companies’ mission and values and adhering to the Corporate Code of Conduct. 13. Maintains high regard for member privacy in accordance with the corporate privacy policies and procedures. 14. Regular and reliable attendance is required
Minimum Qualifications 1. BS / MS / DPT degree from an accredited Physical Therapy school with current NYS licensure. 2. Minimum of 2 years experience in an acute care facility and/or an acute intermediate rehabilitation facility. 3. Possession of a valid NYS motor vehicle operator’s license. 4. Pre-employment physical examination and an annual health assessment as require by New York State Health Department.
