This position is responsible for direct nursing care for certified agency patients. This position may function as the primary nurse. The RN is responsible for the delivery of patient care. This involves the assessment of patient and family needs, and the development, implementation and evaluation of an appropriate plan of nursing care. This RN also serves as the case manager, which involves the identification of appropriate services, supervision of home health aide staff and ongoing review and evaluation of the utilization of services.
Graduate of an accredited school of registered professional nursing. BSN preferred. Valid NYS registered nursing license. Minimum of 2 years (recent) experience as a licensed RN in a home health or acute care setting. Must have ability to travel (own car and valid NYS drivers license required). Demonstrates strong computer, interpersonal, communication, organizational and time management skills.
